Request for Letter of Accommodation Form (W128)

Students registered with Student Accessibility Services (SAS) with a permanent disability, who DO NOT NEED changes to their accommodations, should complete this form to request their Letter of Accommodation.

If you have a temporary disability, or would like to discuss changes to your accommodations, contact our office at 416-736-5383 to schedule an appointment with your Accessibility Counsellor.

Please Note: we cannot generate letters for students registered with the Accessibility, Wellness, Counselling Centre (AWC) at Glendon. Glendon students should contact AWC to request their letters.
