Our name is changing. Welcome to Student Accessibility Services (SAS)! We are looking forward to working with you! Please complete this questionnaire once you have accepted an offer of admission from, and been admitted to, York University. Please note, you will not be able to submit this form without uploading disability related documentation.

Glendon students should contact Accessibility, Well-Being, and Counseling (AWC) at Glendon to find out about registration.

The information provided in this questionnaire will help us to determine what supports you might need while studying at York. This form will likely take 30 to 40 minutes to complete - we want to get a clear sense of what your needs might be.

YOUR DISABILITY RELATED DOCUMENTATION MUST BE UPLOADED TO THIS REGISTRATION FORM (information about how to do this is provided below). We are usually able to schedule students to take part in a Registration Session within 2-3 weeks of receiving their registration information.

If you have not heard from us within 3-4 business days of submitting your registration information, please email or call 416-736-2100 x 77424.

PLEASE NOTE: students who have met with an Accessibility Counsellor should submit new, updated or additional documentation to their SAS Counsellor, it is not necessary to complete this questionnaire again. Students returning to York after some time away, who were previously registered with Disability Services should contact our office to find out about reactivating their file (it might not be necessary to complete this questionnaire).
