Accommodation Letters Request Form Copy

Student registered with Glendon Accessibility Service with a permanent disability, who DO NOT NEED changes to their accommodations, must complete this form to request their accommodation letters.

If you have a temporary disability, or would like to discuss changes to your accommodations, contact the AWC centre at 416-487-6709 to schedle an appointment with your accessibility counsellor.

Once your request for a letter of accommodation has been received, your file will be reviewed and request will be proceed. Your letter of accommodation will be generated and emailed to you within 5 business days AFTER you submit the request.
